1. Registration fees must be paid in full and MUST accompany the Registration Form. Registrations and/or payment WILL NOT be accepted on site
2. Early bird registrations will be accepted until February 13, 2009 at 4:00pm MST. After this date, registrations will be subject to the regular conference fee.
3. Regular registrations will be accepted from February 13 – April 10, 2009 at 4:00pm MST.
4. Notice of cancellation must be made in writing to Denni Wentzell at the Brain Injury Association of Alberta (see address below). Cancellations received by April 1, 2009 will be subject to a $75 cancellation fee. No refunds will be issued after April 1, 2009.
5. Notice of substitutions must be made in writing to Denni Wentzell by April 1, 2009, and must include all pertinent registration information.
6. Conference registration may be made by Cheque, Money Order, VISA or MASTERCARD.
7. If you are paying by cheque, please make all cheques payable to Brain Injury Association of Alberta. Cheques and Money Orders should be mailed to the BIAA office (address listed below).
8. Registration confirmation will be sent to you via email or through Canada Post.
9. A payment receipt will be sent to you via email or through Canada Post.
10. NSF (non-sufficient funds) cheques will be assessed a $30 service fee. Payment of service fee and conference registration will be accepted via Money Order or Certified cheque.

AUTHORIZATION TO USE WRITTEN MATERIALS / PHOTOGRAPHS

I, hereby authorize and acknowledge that the Brain Injury Association of Alberta may be videotaping and photographing various components of the conference and may use, reproduce, and/or publish photographs that may pertain to me. I understand that this material may be used in various publications, public affairs releases, recruitment materials, or for other related endeavours. This material may also appear on the Associations Web Page. The Association may publish materials, use my name, photograph, and/or make reference to me in any manner that the Association deems appropriate in order to promote/publicize the conference.
